TIM offers the widest network with the best reach into rural and mountain areas.
Italy has strong 4G everywhere tourists go and a 5G network that is now broad across Rome, Milan, Florence, Naples and the main northern cities. TIM and Vodafone carry the deepest coverage, including the Amalfi coast, the lakes and most of Tuscany. The familiar weak spots are the Dolomites and the Apennine interior, the smaller Sicilian and Sardinian hill towns, and some coastal train lines that cut through tunnels. For ordinary city and coastal travel, signal is reliable and fast.
Typical local SIM: $15-25. Usually sold at operator stores in any town, plus tabacchi and electronics shops; airport counters at Rome Fiumicino and Milan Malpensa. Italian law requires passport identity registration to activate a SIM, which can mean a short wait in store. EU carrier terms is included on most prepaid plans, so one Italian or EU eSIM covers an onward trip through neighbouring countries under the carrier terms.
